Note: The Sponsor Recommendation Form is no longer a required part of the application. It is recommended that you have your sponsor submit a supporting story or comments on your behalf to: support@serviceawards.org, but it is no longer a requirement for the application.

Use Facebook & Twitter to share your online service profile and get supporters to vote!
Applications will be accepted between December 15, 2010 through February 7th, 2011.
Along with the student application, the student will be asked to submit 3-6 photos and/or one video (less than 3 minutes) about the student and their service project. Applicants are encouraged to collaborate with the video production or communications departments on their campus to help create video content about their project.

Important notes regarding photos/video:
- - When selecting photos or videos of your service work that include other people, please be considerate as to whether there are any circumstances under which some of the individuals would not want to be identified. While we realize it is difficult to obtain photo or video permissions from everyone visible, we ask you not to submit images of individuals who may need to keep their identity confidential due to the nature of the community services they are receiving. We appreciate you using your best judgment, and if any individuals shown in your photos or videos have any concerns, we are happy to remove them from the site immediately.
- - If the service project is part of an AmeriCorps, Learn and Serve, or VISTA program, please try to include photos/video shots with the logo.
Applicants are highly encouraged to submit stories written in support of the applicant and/or their service project. Such stories can be written by parents, mentors, professors, friends, classmate, co-workers, etc.
You can create awareness about your Participant Profile page on the SISA website by posting a message on your Facebook Profile, and creating a Page or Group. You can ask others to read your submission and vote for you, and also post on other people’s pages when it’s appropriate to do so. Ask your friends and family to get involved and do the same. Send out invitations to your friends to "LIKE" your Page and "JOIN" your group.
Use Twitter to send out a few tweets announcing your participation in SISA and direct followers to your Participant Profile page on the SISA website.
